Case Management Services in line with Personalisation Agenda

We are currently revamping our case management services to more accurately reflect the current Department of Health policy regarding the personalisation agenda.


We are a Multi-Disciplinary Team of Case Managers which includes occupational therapists, social workers and nurses.  We also have access to speech and language therapist, physiotherapists and psychologists.
When a case comes in it is allocated to the most appropriate professional to be involved as the Lead Case Manager.  We also have a policy of having a shadow Case Manager for each case so that the client always has a point of contact with someone who knows their case.

We are fully committed to Outcome Focused Care Planning which enables the client to set and have control over their own goals and outcomes.  We take into consideration all aspects of the client’s life and include family carers in the assessment process.

We believe that “actions speak louder than words” and that our dynamic approach leads to effective continuity.
We all have extensive experience working in both the health and social care sectors.  As well as case management being part of our work, we have a number of contracts with local social services departments and current Primary care Trusts to carry out specialist manual handling risk assessments, complex occupational therapy assessments, manual handling and social care training as well as carrying out clinical negligence reports, judicial reviews etc.

We believe this gives us the edge over other Case Management organisations as we are involved with current practice within social care and indeed are often involved in helping to develop practice.
